Personal Physicians

Dr. Geoffrey Litner

M.D., CCFP, M.B.A. HCM

Dr. Litner graduated medical school with honours from the University of Toronto where he was recipient of multiple awards for clinical proficiency and academic merit. He went on to complete his medical training at Women’s College Hospital, achieving Board certification in Family Medicine. Dr. Litner’s interest in medical administration and clinic operations led him to complete an MBA in Healthcare Management at the University of Phoenix in 2003. Over the course of his career, Dr. Litner has served in teaching and administrative capacities in multiple settings both within the community and within the Family Medicine Department at Women’s College Hospital. With a background in clinical epidemiology, Dr. Litner possesses a keen interest in evidence-based practice and has published numerous research articles in peer-reviewed journals.

Dr. Lance Ceresne

B.A.Sc., M.D., CCFP

Dr. Ceresne graduated from Engineering Science at the University of Toronto and went on to study MR angiography in the department of Biophysics at Stanford University. He subsequently returned to Toronto where he completed his medical training and achieved Board certification in Family Medicine. Prior to joining MDDirect, he was occupied in the Family Medicine Department at Sunnybrook Health Sciences Centre, where he served as Family Medicine Residency Program Director, supervising the medical education of countless residents and medical students. His other responsibilities included coordinating Family Medicine Rounds, assisting in the development of an Office-based Clinical Procedures program, and delivering numerous lectures and seminars. Dr. Ceresne has received awards in both teaching and resident advocacy from the University of Toronto’s Department of Family and Community Medicine.

Dr. Graeme Schwindt

MD, PhD, CCFP

Dr. Schwindt completed his undergraduate and medical education at the University of Toronto. He is a graduate of the combined MD-PhD program. While training he received multiple awards for his clinical and research work, which focused on Alzheimer’s Disease and brain imaging techniques at Sunnybrook Health Sciences Centre. He has authored numerous peer-reviewed publications. Dr. Schwindt then completed family medicine training at Sunnybrook where he received the Stan Kandel award for excellence in patient care as a resident physician. His focus is on cultivating a strong doctor-patient relationship, providing comprehensive patient centered care, and he maintains an academic interest in aging and dementia research.

Dr. Anisa Janmohamed

B.Sc., M.D., CCFP

Dr. Janmohamed attended Simon Fraser University on a full scholarship and graduated with a Bachelor of Science, Biology. She went on to study Medicine at the University of British Columbia where she was the recipient of numerous academic awards. She moved east to complete her residency in Family Medicine at Queen’s University, and during her training worked with a family practice in Cuba providing outreach services to patients in the community. Prior to joining MDDirect, she ran a successful family practice in Toronto for over a decade, and has devoted her career to patient care and advocacy. She works closely with her patients to educate and inform them, and ensure that they have an active voice in their care. Additionally, Dr. Janmohamed continues to work as a member of a surgical team with Courtesy Privileges at Toronto East General Hospital and Women’s College Hospital.

Dr. Kirstin Theman

B.Sc., M.D., CCFP

Dr. Theman grew up in Victoria, British Columbia where she attended the University of Victoria, earning a Bachelor of Science with Distinction in Biology. She subsequently moved east to study Medicine and completed her undergraduate medical training and residency in Family Medicine at Queen’s University. Since then, Dr. Theman has worked in various clinical settings on both sides of the country, most recently as a member of the Toronto-based, multidisciplinary, Bridgepoint Family Health Team. She is a devoted and skillful family practitioner who enjoys caring for patients of all ages and backgrounds, helping them to navigate the medical system and improve their lives, while managing a broad array of health concerns. She is extremely caring and accessible and puts her formidable talents to use in the service of her patients at MDDirect.

Dr. Sherri Farber

B.A., M.D., CCFP

Dr. Farber was born and raised in Toronto. She received her B.A. in International Development Studies from Dalhousie University and her Medical Degree from Ben Gurion University in Israel. She completed her Family Medicine residency at Stamford Hospital/Columbia University in the United States. She is the recipient of multiple awards, including the Gold Humanism award and the ‘Excellence in Teaching’ award. Since moving back to Toronto in 2016, Dr. Farber has worked in a variety of clinical settings, including Family Practice, Urgent Care, Executive Healthcare and Emergency Medicine. She has a special interest in Obesity and Weight Management. Dr. Farber is passionate about patient-centred care and collaborative medicine. She takes a keen interest in her patients’ lives and advocates for all aspects of their wellness.

Dr. Andrew Chou

B.Sc., M.D., CCFP

Dr. Chou grew up in Vancouver and Ottawa where he graduated summa cum laude from the University of Ottawa’s Biomedical Science program. He went on to study medicine at the University of Toronto, where he graduated with Honours, before completing his residency in family medicine at the Toronto Western Hospital. Prior to joining MDDirect, Dr. Chou ran a practice for over seven years at a pioneering Toronto clinic, leveraging advances in communications technology to connect directly with patients while minimizing wait times. He also provided care and support to students and staff at one of Canada’s most diverse universities. Dr. Chou believes strongly in a compassionate, accessible, and patient-centred approach to healthcare. He believes that effective medicine means not only skill, knowledge and communication, but also taking the time to build a relationship of trust with each and every patient.

Dr. Ananya Raghavan

M.D., CCFP

Dr. Raghavan graduated from the Health Sciences program and completed her medical degree at McMaster University. She went on to complete residency training at the University of Toronto, Women’s College Hospital, achieving Board certification in Family Medicine. During her education, she had the opportunity to train in most of Toronto’s major hospitals, as well as in both public and private institutions in Uganda. Her diverse experience has allowed her to cultivate her passion for family medicine. Prior to joining MDDirect, Dr. Raghavan ran her own successful family practice for almost a decade, where she took great pride in building rich and fruitful relationships with her patients. She is a strong believer in autonomy and works diligently with each of her patients to help them achieve their health goals.

Dr. Erika Frasca

M.D., CCFP

Dr. Frasca grew up in Mississauga, Ontario. She attended McGill University on a full scholarship, graduating with First Class Honours in Physiology. She then completed medical school at the University of Toronto, garnering awards for both high academic standing and research. Her Family Medicine Residency at Women’s College Hospital included clinical training at Toronto’s academic hospitals and at several remote rural locations (Kashechewan, Moose Factory, Parry Sound), culminating with a fellowship in Low Risk Obstetrics at Trillium Hospital. Prior to joining MDDirect, Dr. Frasca was the co-lead physician for a large interdisciplinary Family Health Team in downtown Toronto. She has a passion for comprehensive wellness and strongly believes that the patient-physician relationship is the foundation to good health care.

Dr. Naomi Nohara

M.D., CCFP

After growing up in Calgary, Dr. Nohara moved to Montreal to complete a Bachelor of Science in Anatomy and Cell Biology at McGill University. She then obtained her medical degree at the University of Western Ontario before finishing her residency training in Family Medicine at the University of Toronto. Since graduating, Dr. Nohara has practiced in a variety of medical models caring for a diverse population of patients. Prior to joining MDDirect, she worked in her own family practice for over a decade. Dr. Nohara is passionate about Family Medicine and dedicated to providing holistic, patient-centered care that is individually tailored to each of her patient’s unique needs, values, and wellness goals.

Dr. Jeffrey Chan (Tai Kong)

M.D., CCFP

Dr Chan was born and raised in Montreal where he graduated with a Bachelor of Science in Physiology at McGill University. He went on to earn his medical degree and complete his residency training in Family Medicine at McGill. Prior to joining MDDirect, Dr. Chan worked in a downtown Toronto family practice providing empathetic and comprehensive care to a diverse group of patients. He is a strong advocate for patient autonomy as part of a healthy doctor-patient relationship, and is fiercely dedicated to helping his patients cultivate a healthier future.

Dr. Holly Lam

BHSc, MD, CCFP

Dr. Lam was born and raised in Toronto. She graduated summa cum laude from McMaster University with a Bachelors of Health Science. She remained at McMaster to complete her Medical Degree before relocating to Women’s College Hospital in Toronto where she trained in Family Medicine. Prior to joining MDDirect, Dr. Lam ran a successful Toronto family practice for over a decade where she cultivated strong therapeutic relationships with patients of all ages and backgrounds. She believes that individual empowerment and comprehensive personalized care are key to helping her patients achieve their health goals.
